End of Life Care

SwanCare’s residential aged care teams are trained to be able to provide high-quality end of life care with the support of the Metropolitan Palliative Care Consultancy Service.

Support to families is also an important part of the care provided at SwanCare.  Comfort for those going through a difficult time is readily available, as well as bereavement support to families and friends.  Beds are also obtainable for loved ones wanting to stay overnight during this period.

Planning for Future Care Needs

Existing SwanCare aged care residents requiring palliative or end of life care do not need to be transferred elsewhere, as SwanCare is equipped to provide the appropriate care within all four residential aged care centres.

This ensures continuous care is provided within a familiar and comfortable environment for your loved one.

For those living independently, SwanCare At Home can assist those requiring care, support, and equipment within the home environment.
