Phil is a values driven leader dedicated to improving lives through quality care and innovation. With extensive expertise in operations, corporate and clinical governance, risk management, and strategic planning, Phil has consistently worked to support and improve the healthcare sector.

Phil began his healthcare career as a Paramedic and held various clinical and senior leadership roles during his 14-year tenure at St John Ambulance. In 2019, he transitioned into the aged care sector, assuming senior leadership and executive roles that have shaped organisational growth and service delivery.

Phil holds post-graduate qualifications in health leadership and emergency management and is a graduate of the Institute of Company Directors and Leadership WA. As the CEO of SwanCare, Phil is committed to fostering a vibrant culture, enhancing client experiences, and establishing SwanCare as a trusted leader in positive aging services.

Jae Smith

Chief Governance & Strategy Officer

Jae is a purpose-driven executive transforming aged care through strategic leadership, governance and system reform. With over 15 years’ experience in health and aged care most recently as General Manager at MercyCare, she brings operational expertise, a commitment to process improvement and a focus on innovation.

Jae leads major transformation initiatives, from aged care reform, compliance and executive alignment. Her leadership is grounded in clarity, compassion and a commitment to building systems that genuinely serve people.

A Certified Chair and graduate of the Governance Institute of Australia, Jae brings strong governance credentials and values-based leadership, honed through LeadershipWA. She champions innovation in digital capability, AI and inclusive care, earning national recognition for her impact.

Respected for her cross-sector fluency and ability to lead through complexity, including the COVID-19 pandemic and landmark reforms, Jae is known for bringing clarity to ambiguity and driving lasting change.

Nicki Washington

Nicki brings deep operational expertise across aged care, disability, and health insurance. With qualifications in leadership, business and finance, and as a graduate of the Australian Institute of Company Directors (AICD), she is passionate about contributing to purpose-driven organisations that enhance lives through comfort, dignity and care.

Nicki has a proven track record of delivering strategic operational innovations that improve service delivery and foster positive engagement and drive excellence.

At SwanCare, she is focused on building sustainable, people-centred systems that support continuous improvement , deliver high-quality service and exceptional outcomes for residents and staff.

Anne Hansen

Chief People & Culture Officer

Anne is a Human Resources specialist with a strong track record of partnering with CEOs to shape innovative people strategies that align with organisational goals and drive meaningful outcomes.

As a graduate of the Australian Institute of Company Directors and with qualifications in Human Resources Management, Anne brings deep expertise in high-performance program development, succession planning, employee relations, change management and organisational transformation.

Her career spans both the mining and disability sectors, where she has consistently championed workforce wellbeing through care, clarity and purpose. At SwanCare, Anne is committed to ensuring human connection remains central; empowering individuals to thrive and deliver their best in service of others.

Darren Sumner

Chief Corporate Services Officer

Darren brings a wealth of experience from a dynamic career spanning logistics, FMCG, manufacturing, retail and the not-for-profit sector. After years driving commercial success, he transitioned into purpose-led roles supporting disability, mental health and aged care services. His work has helped improve systems, streamline processes and empower teams to deliver more impactful services.

Holding a Master of Business Administration, CPA accreditation and as a graduate of the Australian Institute of Company Directors, Darren is passionate about strengthening service delivery through operational excellence and strategic leadership. At SwanCare, he is focused on building sustainable, collaborative solutions that enable the organisation to continue fulfilling its mission with integrity and care.
